Be sure to include your 's complete name, EIN, mailing address and state the changes you would like to make so that the department can correctly identify your IRS account.

Confirmation
The IRS will send a letter confirming the receipt of the cancellation. If the entity has not received the confirmation letter within 60 days, it should fax a copy of the original letter, annotated "Second Request", to the campus that services the entity's state.
